QUALIFICATIONS
SSD/CNA Certification
OBJECTIVES
To assist residents with basic needs, coordinate social services, and act as representatives for residents who are unable to advocate for themselves.
FUNCTIONS
Comply with the Facility’s personnel and safety policies and procedures.
Coordinates admissions process, including readmissions and room changes.
Ensures positive admission experience for residents and families.
Prepare initial and quarterly resident assessments for the medical record and MDS.
Coordinate and support the operation of a Residents’ Council.
Participate in resident care plans.
Promote and facilitate resident rights.
Receive resident/family grievances and complaints and facilitate satisfactory resolution.
Provide information to resident/families as to Medicare/Medicaid, VA, and other financial assistance programs available to the resident.
Assist residents/families in preparation of Medicaid applications.
Record and maintain regular Social Service progress notes indicating response to the treatment plan and/or adjustment to institutional life.
Assist in interpreting social, psychological, and emotional needs of the resident/family to other resident care team members.
Develop and maintain a good working rapport with other departments within the facility, and outside community health, welfare, and social agencies, to assure that social service programs can be properly maintained to meet the needs of the residents.
Assist resident with information concerning resident rights, living wills, durable power of attorneys, etc.
Job Type: Full-time
Pay: $20.00 - $24.00 per hour
