Skilled Nursing Facility seeking a Social Services Director.

Our Skilled Nursing Facility offers long / short-term care, secured memory care, rehabilitation, respite services, bariatric care, and Inpatient therapy.

OVERVIEW :

The social services director will work with residents in the facility to identify their psychosocial, mental, and emotional needs, along with providing, developing, and / or aiding in accessing services to meet those needs. In addition, the SSD is responsible for fostering a climate that enables residents to maximize their individuality, independence, and dignity. This shall provide residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.

The ideal candidate will possess the following :

1 year of social services experience in a Skilled Nursing or Long-Term Care setting is preferred.

Manage and direct the Social Services Department and identify and provide for each resident’s social, emotional, and psychological needs.

Manage department to assure all psychological and psychosocial needs of residents are met

Understand the MDS process with the ability to interview, input, and complete social service assessments

Accurately assess, document, and provide interventions to meet the psychosocial needs of all residents; develop care plans based on assessments

Provide effective facilitation of resident care conferences : schedule, send letters to families / responsible parties, and participate in care conferences

Facilitate all resident discharges (arrange for home health, DME, assist with transfer to other facilities)
