What are the responsibilities and job description for the Director of Admissions position at Long term Healthcare Facility?
Our facility is actively looking for a Director of Admissions to join our administrative team. As the Director of Admissions, you will be responsible for establishing referral sources and to obtain required information and admit residents in an efficient manner.
You will assist to build relationships with area case managers, physicians, social workers, community organizations and hospitals to increase census and highlight the quality of care and services provided in our facility.
- Markets programs and facilities by preparing informational brochures; writing and placing answering questions; conducting tours.
- Coordinates all admissions and providing advertisements;
- Screens patients by comparing patient's condition to admission criteria; PRI and PASSAR / evaluates, accepts, or rejects patients after consulting with management team.
- Obtains applicant information by requesting completed applications and medical information; verifying and clarifying information; interviewing patients and family members; explaining admission criteria.
- Admits patients by completing admission and financial responsibility forms; coordinating and arranging physical, social, emotional, and support service requirements.
- Determines the residents' eligibility for assistance provided by third party payers, such as, but not limited to: Federal, State, Community, and Veteran's Agencies and obtains prior approval from the sponsoring agency when required.
- Upon admission, greets new resident and shows them to their room. Provides the resident with facility welcome bag and answers any questions they may have upon arrival.
- Upon admission, may assist Social Services in their review with the resident / family / friends all pertinent admission information such as Residents' Rights, Facility Rules and Regulation, Bed Reservation Policy, and Transfers and Discharges.
- Resolves patient/family dissatisfactions by investigating concerns; recommending changes in service policies and procedures.
- Makes appropriate referrals to other facilities when resident transfer is recommended due to need for assistance or services that cannot be provided by the facility when a resident/ family requests a transfer.
- Keeps professional matters confidential.
- Any additional Admission-Marketing tasks as assigned by Administrator.
