Completion of minimum data set assessments with accuracy and attention to detail.Creation of comprehensive care plans based on a thorough review of the patient's medical record.Effective communication with direct care staff, physicians, and family members to ensure coordinated and personalized care.Development and completion of the patient assessment process in compliance with federal and state regulations, as well as company policies and procedures.Coordination of MDS systems, resident assessment, and timely care plan implementation.Strong knowledge of nursing and medical practices, procedures, and relevant laws, regulations, and guidelines in the long-term care setting.Skills and Abilities :
Strong clinical assessment skills to ensure accurate and thorough evaluations.Ability to make independent decisions regularly, demonstrating sound judgment and critical thinking.Effective interpersonal skills and the ability to work collaboratively within an interdisciplinary team.Basic computer skills for documentation and data management.Proven written and oral communication skills for clear and concise reporting.Strong decision-making and analytical abilities to address complex patient needs.Qualifications :
Current, unencumbered Nursing license in the state.Proficiency in reading, writing, speaking, and understanding the English language.Previous experience in a skilled nursing and rehabilitation setting.Strong clinical skills and a comprehensive understanding of the MDS process.Minimum of 1 year working in a skilled nursing facility.Education :
