JOB SUMMARY: The Corporate I-SNP (Institutional Special Needs Plan) Patient Liaison is an employee of our partner SNF (Skilled Nursing Facility) organization. This role functions within assigned partner SNFs and is responsible for supporting the Longevity I-SNP. The Patient Liaison is responsible for I-SNP strategy, assisting the SNF with enrollments and ensuring compliance across the partner organization. This role works with SNF leadership to identify appropriate members and assists with contacting the patient or responsible party regarding the I-SNP Program and its benefits. The Patient Liaison will coordinate all communication between Longevity and our partner organization including but not limited to membership, utilization, contracting and quality outcomes. The Patient Liaison will lead education initiatives with facility staff and providers as needed to support the operational success of the Longevity ISNP. DUTIES/RESPONSIBILITIES: Serve as a liaison between Longevity, our partner organization, SNF facility leadership, and SNF patients/responsible parties. Establish facility-specific I-SNP strategy and enrollment goals and monitor progress ongoing. Maintain workplan for assigned facilities and be responsible for the ongoing management of I-SNP product at the assigned facilities. Serve as a champion for I-SNP program, engaging and educating SNF leadership and providers as needed. Assist SNF with review of eligible patients lists, focusing on new long-term care patients. Outreach to patient and/or responsible party and provide initial I-SNP education. Communicate I-SNP referrals to Longevity in a timely manner. Review outcomes data across partner organization and within individual assigned SNFs. Coordinate with partner organization to address identified quality and cost concerns. Support organization's strict adherence to the Company's Compliance Plan and Standards of Conduct with emphasis on compliance with established procedures and protocol, internal controls, and maintaining the highest level of workplace behavior. Enforce policies and programs that encourage employees and related entities to report suspected fraud and other improprieties without fear of retaliation. Support the compliance program in identifying and responding to compliance deficiencies and corrective action Plan. Supervise and execute special projects as assigned. Performs other related duties as assigned. Education and Training: High school diploma or equivalent required. Bachelor’s degree or equivalent work experience in Social Services or Case Management preferred. Knowledge and Experience: Relevant Work Experience: 3 years or more of skilled nursing experience. Excellent communication and presentation skills to all levels of staff and management. Strong project management and coordination skills.

Salary : $49,000 - $74,000
