Key Tasks and Responsibilities:
- Clinical Team Leadership
- Lead recruitment, onboarding, and ongoing management of field-based nurse practitioners
- Develop and implement clinical protocols and workflows
- Provide mentorship and professional development for clinical team
- Monitor and optimize clinical team performance
- Facility & Partner Relations
- Collaborate with partner medical groups on clinical initiatives
- Ensure smooth clinical operations and services across all care settings
- Quality & Risk Management
- Drive documentation accuracy and completeness
- Lead quality improvement initiatives
- Oversee risk adjustment activities
- Implement best practices for medical expense management
- Program Development
- Design and implement clinical programs to improve patient outcomes
- Develop training materials and protocols as needed
- Establish metrics and monitoring systems
- Lead continuous improvement efforts
- Performs other related duties as assigned.
Supervisory Responsibilities:
- Lead recruitment, onboarding, and ongoing management of field-based nurse practitioners and other clinical staff
Credentials & Coverage:
- Valid state driver’s license with a good driving record and proof of automobile insurance per minimum required by home state.
- Auto insurance coverage as required by your state of residence. New employees will be responsible for providing proof of coverage prior to hire and on an ongoing basis.
Education and Training:
- Clinical experience not required but a major plus
Knowledge and Experience:
- 5 years’ experience managing Clinicians, bonus if field-based clinicians
- Management experience in value-based care setting, bonus if ACO
- Management experience in Fee-For-Service Medicare practice
- Experience with SNF/ALF patient population
- Strong understanding of risk adjustment and quality metrics
- Excellent communication and relationship management skills
- Ability to travel up to 75% of time
