We are currently seeking a skilled and compassionate Advanced Practice Provider (APP) to join our primary care team.
Position Overview:
We are looking for an enthusiastic and experienced Advanced Practice Provider (NP or PA) to join our growing primary care team. As an APP, you will play a critical role in delivering high-quality care to our patients. You will work closely with our physicians, specialists, and other healthcare providers to ensure optimal health outcomes for our patients. The ideal candidate will have a strong clinical background in primary care and a passion for building long-term relationships with patients.
Key Responsibilities:
- Provide comprehensive primary care services to age-appropriate patients, including assessment, diagnosis, treatment, and follow-up care
- Perform routine physical exams, screenings, and preventative health services
- Develop and manage treatment plans, including prescribing medications and therapies
- Collaborate with physicians and specialists to provide coordinated care for complex patients
- Educate patients and their families on health promotion, disease prevention, and lifestyle modifications
- Document patient encounters in the electronic medical record (EMR) accurately and efficiently
- Maintain up-to-date knowledge of best practices and evidence-based care
- Participate in continuous quality improvement initiatives and contribute to a positive work environment
Qualifications:
- Active certification as a Nurse Practitioner (NP) or Physician Assistant (PA) with the appropriate state licensure
- Previous experience in primary care or a related field preferred
- Strong clinical skills with the ability to diagnose and manage a wide range of medical conditions
- Excellent communication and interpersonal skills, with the ability to work well with patients, families, and team members
