A prominent Home Health provider with locations throughout West Virginia is currently recruiting a Home Health Network Liaison to facilitate admissions from assigned hospitals, clinics, and MD offices into our agencies. You will match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into our home health network to meet / exceed census and goals. This Morgantown, WV based liaison position has opportunities to grow into a more regional role.
Responsibilities will include :
- Communicating with the Interdisciplinary team daily to ascertain information and knowledge about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events
- Managing the electronic referral system for assigned centers and develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
- Completing clinical reviews via Allscripts and / or onsite evaluation as needed or requested, and coordinating a seamless transition to assigned locations
- Remaining visible at assigned hospital and being available as a resource to promote the home health agency in a positive manner
- Abiding to all HIPPA laws and observing confidentiality policy
- Presenting the benefits of the home health agency to patients, families, hospital employees, etc.
