What are the responsibilities and job description for the DIRECTOR OF PATIENT CARE SERVICES position at Lorain County Health & Dentistry?
PRIMARY PURPOSE
The Director of Patient Care Services (DPCS) works in a collaborative role as a leader in a multidisciplinary care team in the ambulatory care setting based on the organization’s patient centered medical home model. The DPCS has responsibility for directing clinical staff, including Manager of Nursing, Nursing Coordinator, Clinical Assistants, Medical Assistants and Care Coordination teams including, Care Managers and Community Health Coaches. The DPCS will assist team members to integrate their work into the larger model of care. The DPCS works closely with the Chief Operating Officer, Chief Medical Officer and Chief Nursing & Quality Officer to assure that clinical operations are efficiently and competently performed at every phase of care delivery. The DPCS facilitates teamwork among clinical support staff as well as between support staff and medical providers. A member of the Core Clinical Quality team the DPCS will have an understanding of the Value Based Care model for each MCO and have the ability to process and manage data to monitor key performance indicators and will strategically direct population health efforts to maximize Value Based Care Incentives. If a barrier to care is identified, this position coordinates care and interventions based on the member’s healthcare needs and goals and follows up with the appropriate supports to attain those goals. The DPCS works closely with the Chief Nursing & Quality Officer and the Quality Manager using various tools to target outreach efforts and coordinate care in response to various indicators including, but not limited to, Managed Care Organization (MCO) reporting, MCO portals, AZARA DRVS reporting, MCO attribution risk stratification. The DPCS facilitates steps to achieve optimal outcomes and as such plays a vital role in coordinating patient care in accordance with LCH&D’s philosophy of “partnership” with patients.
EDUCATION AND CERTIFICATION REQUIREMENTS
- Registered Nurse with active Ohio License.
- BSN required.
SKILL AND EXPERIENCE REQUIREMENTS
- Minimum of Two (2) years experience working in a Community Health Center with working knowledge of value-based care, managed care, population health management, quality / process improvement or project management.
- Must have knowledge of care management and population health principles.
- Motivational Interviewing Training preferred.
- Strong computer skills with proficiencies in Outlook, Word, PowerPoint, and Excel.
- Must have strong organizational skills.
- Ability to work independently with minimal supervision.
- Ability to maintain a calm and professional demeanor and communicate enthusiastically with patients and staff
- Ability to be responsive in working with a culturally-diverse patient population.
- Must have excellent communication skills with the ability to master a therapeutic communication approach.
- Ability to independently coordinate multiple tasks.
- Ability to work cooperatively with others.
- Must have valid driver’s license and satisfactory driving record.
- Bilingual Spanish speaking is a plus.

- Travel between sites may be required.
- Attend relevant professional activities such as organization sponsored events as needed.
