Overview
As a Part-Time Registered Nurse, you will play a vital role in delivering in-home care assessments, developing customized care plans, and ensuring the ongoing quality of care for our clients. You will also supervise and train Personal Care Assistants (PCAs) and Certified Nursing Assistants (CNAs) as needed. Your responsibilities will include conducting quarterly assessments and maintaining accurate documentation in accordance with North Carolina Department of Health and Human Services (NCDHHS) guidelines.
Paid mileage outside of Alamance count.
Key Responsibilities:
- Conduct comprehensive in-home assessments for clients to determine their healthcare needs.
- Develop and implement individualized plans of care based on assessment findings and client preferences.
- Perform quarterly assessments to evaluate the effectiveness of care plans and make necessary adjustments.
- Supervise and train PCAs and, if necessary, oversee CNAs to ensure the delivery of high-quality care.
- Conduct annual evaluations of CNAs to assess performance and provide feedback.
- Complete and maintain accurate documentation of assessments, care plans, and client progress in compliance with NCDHHS guidelines.
- Collaborate with interdisciplinary teams to ensure comprehensive care for clients.
- Provide education and support to clients and their families regarding health management and care options.
Qualifications:
- Valid and active North Carolina Registered Nurse (RN) license.
- Previous experience in home health or community health nursing preferred.
- Strong assessment, critical thinking, and clinical skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficient in documentation and familiarity with electronic health records (EHR) systems.
- Valid driver's license and reliable transportation to travel to client homes.
Compensation and Benefits:
