In 3-6 months we will be looking for the addition of Therapetic Mentors, (Full or Part Time available) who can provide quality, cost-effective care for clients and their families!
The Therapeutic Mentoring (TM) program is a service provided to youth in their home and in other community settings such as school, after school programs, or other community-based environments. TM services include supporting, coaching, modeling, and teaching age-appropriate behaviors, communication, social, problem-solving, and conflict resolution skills, as well as relating appropriately to other youth and adults. The work of the TM is designed to complement and build upon the work being done with the youth by the hub provider (Individual OP Therapy, In Home Therapy, or Intensive Care Coordination).
The Therapeutic Mentor provides structured, one-to-one, strengths-based, culturally competent support to the youth in order to achieve goals specific to daily living, social, behavioral, and communication needs. The TM engages the youth in the following types of activities while working on skill-building: social activities, recreational activities, athletic activities, artistic or creative activities, educational or vacation activities, and activities of daily living based in the community (i.e., applying for a job, learning to take public transportation, etc). The TM attends all treatment planning/care planning meetings for the youth as well as any other meetings such as school meetings as needed. The TM is an outreach position, providing services in the youth’s home and community. Transporting the youth into the community may also be required.

Responsibilities:
- Works one-on-one with the youth in their home and community, helping them to develop and practice the skills needed to achieve their goals as outlined by the hub provider (OP, IHT, or ICC)
- Determines the interventions, activities, and frequency of delivery of TM services to each youth
- Maintains at least weekly contact with the hub provider
- Meets at least 3-4 hours per week (or more as appropriate) with the youth to implement skills-development strategies in a home or community-based setting (outreach)
- Completes notes and all other documentation within required timeframes
- Obtains all insurance authorizations in a timely manner as outlined by each MCE
- Completes all other tasks as required by the program
- Attends weekly supervision, staff meetings, and all other meetings and training as required by the program/organization
- Utilize agency technology in order to complete documents
- Other duties as assigned by supervisor/manager.
Required Skills & Qualifications:
- Must have one of the following:
- High School Diploma/HISET and two (2) two years of experience working with youth
- OR an Associates degree in the human services field from an accredited school and one (1) year of experience working with youth
- OR an Bachelor's degree in the human services field from an accredited university and one (1) year of experience working with youth with mental health needs
- Must be at least the age of 21 or older
- Must have knowledge of community resources and child/family-serving systems
- Must have a valid driver’s license and reliable transportation
- Ability to respond to written and verbal communication in a clear, timely, respectful, and effective manner
- Ability to create and implement strategies to facilitate achievement of agreed-upon goals
- Must value and demonstrate the ability to work with individuals of various backgrounds, be able to explore family culture, and lead discussions about cultural differences and how they may impact the work
- Approved CORI Check
- Proven ability to work both independently and as part of a team
Preferred Skills & Qualifications:
- Community Health Worker Certification
- Bachelors or higher degree
- Bilingual capabilities
