Community Health Worker

Job Overview
The Community Health Worker (CHW) plays a vital role in bridging the gap between healthcare providers and the community. This position involves working closely with individuals and families to promote health education, access to services, and support for those facing health disparities. The CHW will engage with diverse populations, providing essential resources and guidance to improve overall community health outcomes.

Duties

• Serve as a liaison between healthcare providers and community members to facilitate access to health services.
• Educate individuals and families about health-related topics, including disease prevention, nutrition, and wellness practices.
• Collect data on community health needs and assist in the development of programs that address these needs.
• Utilize motivational interviewing techniques to empower clients in making informed health decisions.
• Provide support for individuals with disabilities, ensuring they have access to necessary resources and services.
• Conduct outreach activities to raise awareness about public health initiatives and available services.
• Collaborate with social workers, nurses, and other healthcare professionals to coordinate care for clients.
• Participate in public speaking engagements to promote health education within the community.

Experience
Candidates should possess a background in public health, social work, or related fields. Experience or familiarity with medical terminology is preferred. Skills in data collection and analysis are beneficial for assessing community needs. The ideal candidate will have strong public speaking abilities and experience working with diverse populations, including those with disabilities. A commitment to educating others about health issues is essential for success in this role.

Qualifications:

• Education: High school diploma or equivalent required. A certificate or associate degree in public health, social work, or a related field is often preferred.
• Experience: Prior experience in community health, social services, or healthcare settings is advantageous. On-the-job training is often provided.

Work Environment:

• Some travel may be required for home visits, community outreach, and coordination of services.
• The role may involve working in challenging or underserved areas, with the potential for evening or weekend hours depending on the community's needs.

Physical Requirements:

• The role may involve standing, walking, and occasional lifting of equipment or materials for outreach activities.
• Travel within the community may require use of a vehicle.

Job Types: Full-time, Part-time, Contract

Pay: $17.00 - $22.58 per hour

Expected hours: 30 – 45 per week

Schedule:

• 8 hour shift
• Day shift
