We are seeking a Social Services Assistant to join our team. As the Social Services Assistant you will be working with the Social Services Director in the planning, developing, organizing, implementing, and evaluating of our residents. You will work closely with other healthcare professionals to ensure the highest level of patient care and safety.
Duties :
- The Social Services Assistant assists with planning and implementing a comprehensive Social Services program which provides for counseling and other support services for residents and legal representatives in the facility.
- Identifies medical-related social needs of residents, provides appropriate services to meet the individual, as well as collective needs of residents, and maintains records relating to the residents’ social work needs and care.
- Consults with the Director of Social Services and other department heads regarding interdisciplinary issues, as well as maintenance of appropriate records.
- Works directly with residents and legal representatives experiencing personal and environmental difficulties or concerns related to the customer’s physical or emotional condition.
