Responsibilities
- Develop, implement, and monitor individualized service plans for clients with intellectual disabilities.
- Conduct assessments to evaluate clients' needs, strengths, and areas for improvement.
- Ensure compliance with HIPAA regulations and maintain confidentiality of client information.
- Ensure Home and Community Based Settings Rules are implemented.
- Provide training and support to staff on best practices in memory care, hospice care, and daily living activities (ADLs).
- Provide financial support to individuals served.
- Facilitate communication among clients, families, and interdisciplinary team members to promote effective care coordination.
- Address conflicts that may arise among clients or staff through conflict management strategies.
- Participate in ongoing professional development to stay current with best practices in social work and care for individuals with disabilities.
- Drive to various locations as needed to provide services or attend meetings.
Experience
- A background in social work, mental health, nursing, or related fields is preferred.
- Experience as a CNA is a plus.
- Knowledge of CPR and first aid principles is beneficial.
- Proven ability to manage conflict effectively within a team setting.
- Familiarity with the daily living activities (ADLs) required for supporting individuals with disabilities.
- Ability to communicate effectively it a must.
- Ability to train staff is a plus.
- A valid driver's license is required for this role.
