Essential Functions:
Conducts social evaluation assessments, and locates and refers patients to various social service agencies
Visits patients in the hospital and at home according to program protocols
Distributes literature and assists in teaching pre-natal classes
Examples of Duties
Examples Of Work:
UNDER GENERAL SUPERVISION
Obtains demographic, social, medical, health risk, family environment, employment and financial information in order to enroll patients in pre-natal and resource mothers programs and to conduct a social evaluation assessment survey of pregnant teens and
Locates and refers patients to agencies for shelter, food, supplies, clothing, furniture, counseling, family planning, child support, prescription, examinations, medical or financial assistance; and composes correspondence on the patient's behalf
Conducts infant car seat safety classes, and assists in teaching pre-natal classes to expectant mothers and their families on various topics
Visits new mothers and babies in the hospital, and conducts home visits as prescribed by pre-natal and resource mothers program protocols
Obtains, explains and distributes health education, child care, child development and parenting literature, donated baby supplies and gifts to expectant mothers and their babies
Plans monthly meetings or events for program participants, and solicits merchants, businesses and charitable organizations for program speakers, food and baby supplies, and small gift items
Contacts schools, community agencies and other social service providers to inform them about the pre-natal and resource mother's programs and to gain referrals from them
Completes charts, forms and patient records on the progress of the infant's care, development and immunizations, the expectant mother's knowledge of feeding and care for her baby, and clinic or home visits

Minimum Requirements
Physical Requirements & Working Conditions:
Meets regular and punctual attendance standards and any special needs of the position as determined by management, including mandatory overtime and performing related work
Minimum Requirements:
Associate's degree
Human Services
Social Science
Related field
One year of social service or health service experience and Associate's degree
Other Minimums:
Must provide one's own source of transportation
Equivalency:
High school diploma or the equivalent
Three years of social service or health service experience
An equivalent combination of education and experience may be substituted
