What are the responsibilities and job description for the Licensed Behavioral Health Professional position at Louisville Metro Government?
CHARACTERISTICS OF THE WORK
Under general direction, provides mental health, substance abuse and health behavior change consultation in primary care, pediatrics, women’s health, and dental clinics of Family Health Centers (FHC).
Supports behavioral health integration into health services by working with medical staff to enhance their behavioral health skill set and providing brief, targeted biopsychosocial assessments and follow-up counseling and/or health behavior coaching to adults, children, and families.
EXAMPLES OF THE WORK (These statements are intended to be illustrative of the work of this class.)Conducts behavioral health assessments.
Provides clinical services in the form of brief intermittent psychotherapy, behavioral medicine (relaxation training, weight management, health habits, sleep, hygiene, etc.), crisis intervention and patient education.
Establishes treatment plans with the assistance of medical providers.
Makes referrals to providers and services in the community.
Collaborates with behavioral health and social service staff to enhance the patient’s social safety net.
Participates in program planning and development.
Provides in-service training.
Identifies patients with Limited English Proficiency (LEP) and/or other communication barriers and uses appropriate interpretive services; documents use of interpretive services.
Adheres to standard precautions.
Performs related work as required.MINIMUM REQUIREMENTS
Master’s degree in Social Work or Counseling.
Possession of a Licensed Clinical Social Worker or Licensed Professional Clinical Counselor license from the State of Kentucky.
Three years of clinical social work or counseling experience, which must have included the use of a personal computer for work with internet and word processing software.
An equivalent combination of training and experience may be substituted, as determined applicable by Civil Service.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
Identifies medical record documentation, requests and/or releases.
Lifts and moves materials weighing up to 10 lbs.
Responds to patient and FHC needs via telephone, internet, fax, computer software and in person.
SPECIAL REQUIREMENTS
