This is a grant funded full-time position for one year operating from 8:30-4:00 pm Mondays through Fridays for 32 hours weekly. This position does not supervise other staff. The continuation of this position is contingent upon grant funding.

- Offer a welcoming, warm, and hospitable presence to all clients, staff, volunteers, and visitors.
- Build trusting relationships with clients while enforcing community agreements and maintaining professional boundaries.
- Conduct thorough intake assessments, evaluating clients’ physical, social, and emotional needs.
- Develop individualized care plans using a person-centered and strengths-based approach.
- Provide crisis intervention and de-escalation support as needed.
- Assist clients in applying for Housing Choice Vouchers and other housing opportunities.
- Support clients in procuring and maintaining necessary documentation for housing readiness.
- Advocates for clients and coordinate with property owners and managers and to provide move-in support to facilitate successful housing placements.
- Conduct home visits or community meetings to ensure comprehensive client support.
- Establish and maintain partnerships with mental health providers and other community resources.
- Make referrals to appropriate services and monitor client progress towards goals.
- Assist clients in obtaining medical and financial entitlements and benefits.
- Identify gaps in services and contribute to program development to address unmet needs.
- Maintain accurate and timely client records, including case notes and service plans.
- Utilize the Homeless Management Information System (HMIS) for required documentation.
- Adhere to agency policies, professional ethics, and confidentiality standards.
- Participate in staff meetings, training sessions, and agency events.
- Collaborate effectively with other staff members and community providers.
- Contribute to a positive work environment and support overall program objectives
- Bachelor’s degree in social sciences or closely related field
- 2 years of experience in social services
- Ability to work independently and prioritize effectively in a fast-paced environment with a strong sense of urgency
- Experience providing trauma informed support and interventions to clients while also utilizing de-escalation techniques and
- Very strong organizational and time-management skills
- Intermediate level knowledge of Google products and ability to quickly learn new system programs
- Ability to work effectively in various physical positions, including sitting, standing, walking, bending, and kneeling for extended periods
- Capable of occasional lifting and carrying of supplies up to 25 pounds
- Comfortable navigating a social services environment, which may involve extensive computer use, frequent movement between work areas, and use of stairs
- Ability to work in an environment where domestic animals (e.g., cats and dogs) may be present
- Possess the stamina and flexibility to perform job duties in both office and community settings as required
