Job Description
We are seeking a dedicated and compassionate Clinical Case Manager to join our team. As a member of our acute care department, you will play a critical role in assessing the social, psychosocial, cultural, environmental, and financial situations concerning patients and their families.
Responsibilities
- Assess patients' and families' needs and develop effective care plans to ensure optimal outcomes.
- Promote patient rights and dignity, and involve patients and families in care decisions whenever possible.
- Facilitate smooth transitions between acute, rehabilitative, and long-term care settings to provide continuity of care.
Required Qualifications
- Bachelor's degree in Social Work or Nursing, or Associate's degree in Nursing or Diploma in Nursing.
- Valid NM Nursing license or compact license (within 90 days of hire).
- Basic Life Support certification.
