Overview :

Work as a part-time, day shift Case Management Acute Care Clinical Case Manager (RN) at Lovelace Medical Center in Albuquerque, NM.

Responsibilities :

  • The Clinical Case Manager assesses the patient's and family's social, cultural, environmental, and financial situations.
  • Advocates for patients' rights and dignity.
  • Facilitates discharge processes for optimal care continuum.

    • Qualifications :

  • Minimum : Bachelor's degree in Social Work or Nursing, or Associate's degree in Nursing.
  • Social Worker :
  • Medical Social Worker or Licensed Clinical Social Worker with current New Mexico license and basic life support certification.
  • Registered Nurse :
  • Valid New Mexico nursing license or compact license within 90 days of hire, BLS, and may require ACLS and PALS.

    • Preferred Requirements :

  • Master's degree in Social Work or Nursing preferred.
  • Two years of medical social worker or RN Case Manager experience.
  • CCM or ACM certification preferred.
