Responsibilities
- The RN Navigator is responsible for guiding patients through the healthcare journey. This role involves providing patient-centered education, emotional support, and clinical guidance, ensuring that patients understand their care options and feel supported at each step.
- The Navigator coordinates multidisciplinary appointments, tests, and treatments, serving as a key liaison between patients, healthcare providers, and external resources.
- This role requires thorough clinical assessments, precise documentation, and proactive communication to streamline patient care.
- Additionally, the navigator connects patients to relevant resources to ensure comprehensive and compassionate care through the treatment process.
Qualifications
Job Requirements:
- Associate's Degree from an accredited School of Nursing program.
- Associate's degree from an accredited School of Nursing program.
- State of New Mexico Nursing License or valid multi-state compact license.
- Current CPR certification
Preferred Job Requirements:
- Bachelor's of Nursing or Healthcare Management
- Computer and EMR knowledge
- Patient Navigator and/or Case Management Experience.
