Job Description
Grant-funded position open for a minimum of two years. Evenings and weekends required.
The Community Health Worker (CHW) will participate in a research project, in conjunction with University of Massachusetts, Lowell (UML). The project will focus on enrolling and conducting interventions for at least 100 older adult patients. The majority of the population will be 65 , low income Black, Southeast Asian or Hispanic. The candidate will provide a variety of services to LCHC’s older adult asthmatic patients and their families. Services include but are not limited to asthma education, multiple home visits, case management/follow-up, care coordination and referrals to various resources.
The program’s goal is to improve/ control asthma complications and reduce emergency room visits and hospitalizations for our adult patients with asthma. Outreach efforts will be coordinated with all departments including Adult Medical Services, Family Medicine, Metta Health Center and other agencies in the region. The CHW will provide team based integrated care to deliver improved asthma interventions and individualized home environmental action plans.
The CHW, in collaboration with the integrated team, will develop effective strategies to improve asthma control and decrease urgent care utilization. The CHW will work under the supervision of Program Supervisors at Lowell CHC’s Health Promotion and Adult Medicine departments.
Qualifications
Bachelor’s Degree in Community Health, Public Health or any related field preferred OR equivalent years of Community Health Worker experience. Successful completion of a Community Health Worker formal training program preferred. Written and oral fluency in English and Spanish required. Experience working in a multi-cultural setting. Basic computer skills. Ability to establish positive, supportive and trusting relationships with patients, families and colleagues.
Ability to work collaboratively and effectively with teams. Good oral communication skills, such as listening well, Writing, and using language appropriately. Knowledge of available community resources. Bilingual with ability to read, write and speak in public forums helpful. Candidates with prior knowledge in home visiting and health care fields preferred. Must have reliable transportation.
