What are the responsibilities and job description for the Memory Care Registered Nurse position at Lower Cape Fear LifeCare?
Schedule:
- After your orientation period - your typical schedule will be Monday to Friday, 8 AM to 5 PM.
What you’ll be doing:
- This is a community-based program designed to cater to the unique needs of individuals living with Alzheimer’s disease and other related dementia, and their caregivers.
- Visits will be conducted virtually, in person in the office, and in home visits based on need. The home visits would be wherever the patients consider home (personal residence, assisted living facilities, memory care facilities). We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
- Initial consults last around 2 hours and will consist of taking a complete client history and evaluation of the Palliative Performance Scale.
- Visit frequencies are determined by caregiver and client need.
- You’ll need to be comfortable training the caregivers to care for their loved one living with dementia to provide the proper structure and care to meet their changing needs.
- You’ll use your own personal vehicle, and mileage driven between patient visits is reimbursable.
- There may be some coordination of care between your patient’s community medical provider and other disciplines such as neurology to provide updates.
- Patients are assigned by your manager based on your team’s needs – determined by acuity, geography, and many other factors.
- You’ll perform real-time documentation during visits in the patient’s home.
- Initial visits are scheduled by the administrative team and follow up visits are scheduled by each individual nurse.
You will be part of an Interdisciplinary Team (IDT) consisting of registered nurses, medical social workers, and administrative staff. Volunteers are also available to assist in
JOB SPECIFICATIONS
1. Education: Graduate of an approved school of nursing, either through an AD, Diploma, or BSN program.
2. Licensure / Certifications: Current license to practice professional nursing in the State of North Carolina/South Carolina. Completes Dementia certification within six months and maintains yearly recertification.
3. Experience: Experience preferred in either Home Care, Palliative, or Hospice nursing with a strong background in dementia, Alzheimer’s, or memory care.
