Job Summary:
The Palliative Care LPN/RN provides outpatient clinical support, phone triage for patients and external customers based on appropriate nursing standards, policies and procedures. The patient population includes geriatric patients. The Palliative Care LPN/RN performs other duties as assigned.
Primary Job Duties:
- Competently provides patient care in the clinic setting as well as phone triage based on appropriate nursing standards, policies, and procedures. The LPN works under the supervision of the Director of Community Based Palliative Care.
- Provides reinforcement of appropriate and pertinent patient education to the patient/family according to the Plan of Care to facilitate optimal patient and family comfort and support in life limiting illness.
- Assists palliative providers in ensuring their patient’s medications refill requests are handled in a timely manner, which may include communicating with patients and pharmacies.
- Functions as a clinical expert to centralize and simplify the process of providing the appropriate level of patient care.
- Collaborates effectively with other members of the healthcare team to assure quality of care that is cost effective.
- Appropriately documents and communicates all required information in accordance with agency policies and procedures, accreditation, regulation and legal requirements.
- Annually continues personal development to enhance practice and profession.
- Organizes and performs work effectively and efficiently and can work in an independent manner.
- Maintains and adjusts schedule to enhance team performance.
Job Specifications:
- Education:: Graduate of an approved school of nursing.
- Licensure / Certifications: Current license as a practical nurse in the State of North Carolina / South Carolina.
- Experience: Two years’ experience required. Office/clinic, palliative care or oncology experience preferred.
- Essential Technical / Motor Skills: Ability to speak clearly to communicate with patients, families, physicians and staff regarding patient’s condition and care. Must be able to communicate and be literate in the English language.
- Interpersonal Skills: Strong team membership skills. Strong verbal and written communication skills. Ability to quickly develop positive interactions with team members, clients, customers, and the public. Highly self-motivated and self-disciplined, flexible and positive attitude. Aligns behavior with the needs, priorities and goals of the organization. Works and communicates with internal and external clients and customers to meet their needs in a polite, courteous, and cooperative manner. Committed to quality services. Displays a high level of initiative, effort, and commitment towards completing assignments efficiently. Works with minimal supervision.
- Essential Physical Requirements: Sitting for deskwork and meetings, writing, walking and standing, assisting patients through the rooming process.
- Essential Mental Abilities: Ability to assess client needs for LCFL services, utilize appropriate interventions, evaluate clients’ responses to interventions. Ability to utilize basic mathematics skills. Ability to organize and complete work requirements in a timely manner appropriate to clinic operations and hours of business.
- Essential Sensory Requirements: Ability to use sight to perform to perform clerical duties and documentation. Ability to use hearing to communicate with staff and patients.
- Exposure to Hazards: May encounter exposure to infectious diseases or situations that present a potential threat to personal safety.
- Other - Hours of Work: Monday through Friday 8am-5pm
- Population Served: Adult population with serious, life limiting illness.
