What are the responsibilities and job description for the Substance Use Disorder Counselor position at Lower Eastside Service Center?
BASIC FUNCTION : The SUD Counselor is part of an inter-disciplinary team that will serve 40 single individuals with various needs and histories. SUD Counselor will be responsible for the coordination and services provision of substance and mental health counseling services. The incumbent will maintain community based referral resources to assist with accessing treatment options for residents in conjunction with the inter-disciplinary team made up of the Project Coordinator, Case Managers and Social Worker. The SUD Counselor will oversee recovery readiness, relapse prevention, and recovery planning services; provide individual counseling; coordinate referrals with treatment facilities; educate staff and tenants about approaches to managing substance use; and organize sober recreational activities.
MAJOR DUTIES :
- Performs initial substance use screening of all clients to obtain background information and identifies preliminary substance use needs and develops a preliminary plan of services to meet these needs.
- Conducts a comprehensive substance use assessment, which includes a complete history of substance use, treatment, history of detox / re-hab, drug of choice, and the evaluation of the client's capacity to function.
- Provides on-going individual and group counseling to help clients make appropriate decisions and assists them toward their established goals.
- Functions as an integral member of the inter-disciplinary treatment team and participates in case conferences, team meetings and in-service workshops.
- Develops and implements a formalized curriculum to train clients in workshops and groups regarding relapse prevention, harm-reduction and effect of trauma etc.
- Develops and maintains linkages with community based substance use programs and support service resources.
- Refers clients to appropriate substance use disorder providers in the community.
- Provides follow-up and post-placement support services to clients who enter into treatment or attending a substance use program.
- Provides training and consultation to treatment program staff in substance use disorder principles and practices and helps to insure coordination and integration of substance use disorder within the overall treatment process.
- Maintains concise and accurate case records of client's status and progress. Prepares required report forms and keeps statistics on client contacts, referral and status.
