Job Type: Full-Time - w/ flexible schedule

Loyal Home Health, a leading provider of Home Health Care services, is seeking an experienced Physical Therapist to join our team. This is an excellent opportunity for a skilled therapist to work independently, deliver personalized care, and help patients recover and regain independence in their everyday lives.

Key Responsibilities:

• Provide high-quality physical therapy services to patients in their homes, following personalized treatment plans.
• Assess patient needs, conditions, and goals to develop individualized treatment programs.
• Implement therapeutic interventions to improve mobility, strength, and functional independence.
• Educate patients and their families on techniques, exercises, and safety precautions to optimize recovery.
• Monitor patient progress, modify treatment plans as needed, and report outcomes to the physician and care team.
• Maintain accurate documentation of patient care, progress, and treatment plans in compliance with healthcare regulations.
• Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists, to ensure holistic patient care.
• Foster strong, professional relationships with patients and their families, offering emotional support and encouragement.

Qualifications:

• Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Valid and current Physical Therapy license in IL.
• Minimum of 1 year of experience in physical therapy, with Home Health experience preferred.
• Excellent knowledge of physical therapy techniques, modalities, and equipment.
• Ability to work independently and manage a caseload effectively.
• Strong communication and interpersonal skills to build relationships with patients and multidisciplinary teams.
• Compassionate, patient-centered approach with an emphasis on providing exceptional care.
• Ability to travel to patient homes within a designated area: Lake Forest, IL
• Valid driver’s license and reliable transportation.
