We are seeking a skilled professional to join our team as a Licensed Social Worker – Mental Health. In this role, you will work closely with Veterans and their families to assess their needs and develop personalized plans to achieve optimal well-being.
Your primary responsibility will be to provide covered services to enrolled and unassigned Veterans presenting for care. This will involve conducting bio- psychosocial assessments to determine the needs of Veterans, Veteran's family members and caregivers.
You will work within established standards of social work practice to deliver a full range of clinical social work services. These services may include treatment planning, individual, family and group counseling, VA and community based resource referrals, case management and other services as appropriate.
The ideal candidate will have experience in tailoring interventions for a wide range of psychosocial issues. These issues may include housing/homelessness, food, finances, family/caregiver support, home care/placements, legal, education, employment, disability, mental health/addiction, medical and reporting of abuse/neglect.
You will collaborate with interprofessional teams, members and other programs to ensure seamless treatment planning. Additionally, you will assist with long term care placement, discharge planning, hand off communication and transitions between levels of care.
A Master's degree in Social Work (MSW) from a school accredited by Council on Social Work Education (CSWE) is required for this role. You must also possess a current, full, active, and unrestricted license in the state in which the Outpatient Site of Care.
