Description:

The Patient Navigator, Oncology will guide patients throughout the cancer care continuum, from cancer prevention to treatment, to survivorship, to end-of-life care. The Patient Navigator will work with a multi-disciplinary team of nurses, physicians, and support team members to promote the effective utilization of services and coordination of care for patients within the oncology service line. The navigator will be a liaison between patients, physicians, the hospital, and the community in performing functions that meet the needs of the patients during their treatment.

The Patient Navigator, Oncology will engage patients, create a trusting relationship, and assist in providing coordination and addressing patient needs throughout the patient journey.

Care Coordination & Navigation:

The patient navigator guides patients throughout the cancer care continuum, from cancer prevention to treatment, to survivorship, to end-of-life care. Coordinates appointments with providers to ensure timely delivery of diagnostic and treatment services. Facilitates linkage to follow-up services and community programs that will benefit the patient and caregiver. Serves as a liaison for the patient and the care team.

Provides navigation for patients by addressing any logistic barriers, scheduling complications, childcare needs, etc., that would prevent a patient from showing up at their appointment. Assists patients with connecting with necessary supportive care services, psychosocial and palliative care, and social work.

Patient Engagement & Experience:

Maintains communication with patients, survivors, families, and health care providers (internal & external) at various touchpoints during their cancer care. Monitors patient satisfaction with the cancer experience ad work with the team to address any issues or barriers. Receives and responds to new patient referrals and provide timely follow-up.

Provides advocacy, patient education, and support in accessing community-based and hospital-based programs.

Colleague Engagement & Communication:

Works with the multidisciplinary cancer care team to set goals for patient care. Maintains regular communication with a patient's providers through clinical messages in EPIC and via emails, phone calls, and from cancer care conferences. Acts as a single point of contact among referring providers, patients, and other clinical caregivers to empower patients in dealing with their disease and treatment. Coordinates outreach, educational, screening, and recruitment initiatives across the Loyola Medicine Oncology Service Line.

Organization & Professionalism Skills:

Must have strong self-directed organizational skills and can complete multiple tasks simultaneously, and follow through so that each task is completed. Must act in a professional manner towards internal and external contacts. Will need to meet with patients in-person from time to time, and discuss goals in care plans. Will plan agendas, schedule meetings, support the work and communicate with all team members.

Requirements:

  • Associate's degree in health-related field, or equivalent experience; Bachelor's degree preferred.
  • 2-3 years of previous job-related experience
  • Thorough knowledge of human behavior, social services, community organization, medical terminology, and healthcare systems preferred.
  • Experience with oncology standards of care, current treatment, and survivorship, Knowledge of the breadth and accessibility of community resources, and experience connecting people to appropriate services.
  • Works well with groups and as well as with individuals.
  • CMA, PCT, Nursing, Social Work, Community Health Education preferred.
