Job Description
We are seeking a dedicated Nurse Practitioner to join our team as Primary-Care Providers.
Requirements and Qualifications
- Experience in Family Medicine, Internal Medicine, or Geriatrics preferred.
- New graduates welcome to apply.
- Valid DEA license in Michigan required.
Job Structure
- In-person position with direct commute between home/facilities.
- Work in coordination with an LTC Collaborating Physician.
- Responsible for day-to-day patient care.
- Shared after-hours on-call among all LTC Providers.
Compensation and Benefits
- Productivity-based compensation model (no cap).
- $10k sign-on bonus.
- 15 PTO days annually.
- $2,500 annual CME allowance plus 3 PTO days for CME attendance.

- Monthly phone stipend.
