Responsibilities include:
- Providing day-to-day care to patients, including assessing their needs, developing care plans, and managing chronic conditions
- Collaborating with our team to ensure seamless care transitions and minimize hospital readmissions
- Participating in after-hours on-call rotations and providing emergency care as needed
