What are the responsibilities and job description for the Nurse Practitioner - 10K Sign On position at LTC Provider Care?
LTC Physician Care, PC provides Primary Care (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.
LTC is seeking a dedicated Nurse Practitioner to join our team as Primary-Care Provider to cover Clinton, Madison Heights and Clawson facilities.
Requirements and Qualifications
- Experience in Family Medicine, Internal Medicine, or Geriatrics preferred.
- We are happy to consider new graduates.
- Candidates must hold a valid and unrestricted PA medical license in Michigan.
- Candidates must hold a valid DEA license in the state of Michigan.
- Candidates must have completed a valid U.S. certification program
Job Structure
- This is an in-person position
- Providers will commute directly between home/facilities
- Nurse Practitioners work in coordination with an LTC Collaborating Physician
- Nurse Practitioner is responsible for the day-to-day care of their patients
- After-Hours on-call is shared among all LTC Providers
Compensation and Benefits
- Productivity Based Compensation Model (No Cap)
- 10K Sign ON Bonus
- 15 PTO days annually
- $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
