The purpose of this position is to assist and support the admissions department and social work department in establishing an effective system to gather resident information before and upon admission / discharge to ensure proper placement and initiation of care for the resident. This position will report to the Director of Admissions as well as the Administrator for the Healthcare Center of Lucy Corr.Responsibilities
- Assists admissions department with admitting residents to the facility in a timely and accurate manner and in accordance with established admissions policies and procedures including admissions paperwork and needed information from the patient and families
- Support and assist the discharge planner / social worker with assessments, base line care plans, behavioral assessments, referrals, etc.
- Experience with Point Click Care preferred
- Help / assist social worker with safe discharge planning for patients including ordering equipment, home health, personal care, uniform assessment instruments (UAI), etc.
- Communicates with all residents, guests, volunteers, fellow staff, physicians in a courteous and respectful manner, displaying a positive attitude in all interactions
- Appropriate organizational skills with job tasks and time management
- Provides tours of the facility for potential admissions or general inquiries
- Provides support and timely assistance to foster resolution of any grievances received from staff, residents, and family members / responsible parties
- Assists with clerical work for admissions department and social work department
- Other duties as assignedGeneral Responsibilities
- Performs other related duties as assigned or necessary. Remains flexible and adaptable in work schedules and covering departmental duties when admissions or social work personal are out of the facility.
- Represents Lucy Corr in a manner that portrays professionalism, confidentiality, courtesy, fairness, personal integrity, and respect for othersQualificationsA. Education and / or experience
- Bachelors degree in a related field preferred
- Prior experience in long term care and skilled healthcare services preferred
- Medicare / Medicaid / Managed care experience preferredB. Skills and abilities

- Ability to demonstrate teamwork
