Role Overview:
The Clinical Program Facilitator plays a crucial role in ensuring the successful enrollment of patients into our clinical programs. The Facilitator will work closely with SNF staff, providers, and patients to obtain informed consent and ensure compliance with program requirements. This position requires excellent communication skills, a strong understanding of clinical programs, and the ability to foster relationships with healthcare professionals and patients.
Key Responsibilities:
- Maintain health information in compliance with corporate and federal regulations.
- Patient Enrollment: Work onsite at Skilled Nursing Facilities (SNFs) to assist in the consent process for clinical programs, including CCM and RPM.
- Telepsych & Therapy Facilitation: Coordinate and facilitate weekly tele psych visits and therapy sessions, ensuring patients are prepared for their virtual appointments.
- Patient Education: Provide clear and concise information to patients and their families about the benefits, responsibilities, and details of the clinical programs.
- Collaboration: Partner with SNF staff and providers to identify eligible patients and coordinate the consent process and streamline telepsych and therapy session logistics.
- Support Providers: Serve as the point of contact for providers to facilitate patient enrollment, manage telepsych visits, answer questions, and address concerns.

Qualifications:
- Background 3 or more years in healthcare, patient care coordination, CNA, Unit Clerk, EMT, or clinical facilitation preferred (telehealth experience a plus).
- Strong verbal/written communication and interpersonal skills with the ability to engage patients, families, and clinical teams effectively.
- Knowledge of Skilled Nursing Facilities (SNF) operations and regulations is an advantage.
- Detail-oriented, organized, and proactive in managing patient consent processes.
- Reliable transportation and unrestricted driver’s license
- May be required to have COVID-19/FLU vaccination per the facilities state guidelines.
- Strong analytical and problem-solving skills
- Ability to show strong personal initiative and take ownership over work results.
- High attention to detail, accuracy and follow through.
Rate: $20-$22/hr.
Job Type: Part-time
Pay: $18.00 - $20.00 per hour
