Physician-Per Diem (Hospice)
Lumina Hospice and Palliative Care founded as Benton Hospice Service in 1980, is a community-grown nonprofit, that provides compassionate care and support for individuals, families and caregivers facing serious illness or end of life.
Lumina Hospice & Palliative Care is seeking a per diem (as needed) Physician to join our team. The Physician-Per Diem (Hospice) is responsible for performing all duties in a confidential manner and adheres to privacy practices in accordance with Lumina Hospice & Palliative Care (Lumina) Privacy Policy; additionally, adheres to guidelines of the Lumina Personnel, Safety, and Sexual Abuse policies at all times.
Your responsibilities will be to:
• Is a part of the on call rotation for covering all Lumina lines of business including Hospice, Housecalls, and Palliative Care including after hours and weekends . (phone calls, secure texts from patients/caregivers, EMR inbox, and visits as clinically indicated).
• Assures continuous 24-hour availability of physician phone and secure text coverage when on-service after-hours.
• Takes responsibility for the total care and well-being of all actively enrolled Lumina patients when on-service.
• Coordinates with any ancillary services involved in a patient’s care, as indicated (e.g., home health, community case management, medical specialists, pharmacies, caregiving agencies, etc.).
• Makes appropriate referrals as needed.
• Delegates to RN team during business hours when clinically indicated.
• Supports the Hospice Medical Director as needed (may include face-to-face patient assessments of hospice eligibility).
• Contributes to communication, positive work environment, and team effort.
• Understands and adheres to Lumina Privacy Policy; Lumina Vulnerable Persons Policy; and all policies and procedures contained in the Lumina Employee Handbook.
• Other duties as assigned.
Qualifications:
• Commitment to serving the needs of homebound patients and the terminally ill.
• Practicing Doctor of Medicine, currently licensed to practice medicine in the state of Oregon.
• Actively enrolled Medicare provider.
• Experience managing older adult primary care and common urgent care concerns.
• Experience working in home or long-term-care facility settings (preferred).
• Board certified in Hospice and Palliative Medicine (preferred).
• Comprehensive knowledge and skills in adult medicine; diagnosis and evaluation of patients; and empathic communication of ‘difficult conversations’.
• An understanding and acceptance of hospice philosophy of care.
• An ability to work as a member of a team.
• Current DEA License.
• Current active NPI.
Schedule: Hours are varied. Call coverage and visits per diem as arranged in advance.One weekend a month on-call coverage.
*Preferred applicant will be asked to pass a background check and drug screen following a job offer.

****Must have valid driver’s license and full access to an insured vehicle for work with expectation of significant driving, including in inclement weather conditions.
