- Provide direct patient care services to hospice patients and support the family after normal business hours
- Evaluate patient and family needs and assist in the execution of the patients plan of care
- Document and maintain clinical records
- Attend staff meetings and participate in Quality Improvement initiatives
- Maintains an active nursing license that is in good standing in the state performing job duties.
- Maintains an active CPR certification.
- At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred.
- Proficient in MS Office applications and ability to learn department and job-specific software systems.
- Demonstrate organizational skills. Demonstrate effective verbal and written communication skills. Demonstrate analytical skills when problem-solving. Demonstrate high attention to detail and a high degree of accuracy.
