Job Overview :
The Hospice LPN provides compassionate and skilled nursing care to patients receiving hospice services. The LPN responds to patient needs, provides symptom management, and offers support to patients and families during end-of-life care.
Responsibilities
Provide direct patient care to hospice patients in their homes or hospice facilities.Conduct comprehensive assessments of patient needs, including physical, emotional, and spiritual aspects.Administer medications, treatments, and interventions as prescribed by the hospice interdisciplinary team.Monitor patient vital signs, symptoms, and overall condition, reporting any changes or concerns promptly.Communicate effectively with the hospice interdisciplinary team, including physicians, registered nurses, social workers, and chaplains.Offer emotional support and comfort to patients and families, fostering a caring and compassionate environment.Document all patient care activities accurately and thoroughly in the electronic medical record.Provide education and guidance to patients and families regarding symptom management, medication administration, and end-of-life care.Collaborate with other healthcare providers and community resources to ensure continuity of care for hospice patients.Participate in ongoing education and training to maintain licensure and enhance clinical skills.Requirements :
Graduation from an accredited Licensed Practical Nurse program.Current LPN license in the state of practice.Minimum of one year of nursing experience, preferably in hospice or palliative care.Strong assessment, communication, and interpersonal skills.Ability to work independently and make sound clinical judgments.Compassionate and empathetic approach to patient care.Reliable transportation and a valid driver's license.
