Job Title : Hospice Scheduler
Schedule : M-F 8-5

Job overview :
Scheduling Coordination :
Develop and maintain daily and weekly schedules for hospice staff, including nurses, social workers, and volunteers.
Coordinate shifts and on-call assignments to ensure coverage based on patient needs.
Communication :
Serve as the primary point of contact for hospice staff regarding scheduling inquiries and updates.
Communicate effectively with patients, families, and healthcare providers about scheduled visits and any changes.
Data Management :
Maintain accurate and up-to-date records in the scheduling database.
Ensure compliance with regulatory guidelines related to scheduling and patient care.
Problem-Solving :
Address scheduling conflicts proactively and implement solutions to ensure uninterrupted patient care.
Respond to urgent scheduling requests and adjust schedules as necessary.
Reporting :
Prepare and present scheduling reports to management, highlighting trends and areas for improvement.
Assist in tracking staff attendance and performance related to scheduling.
Requirements :
High school diploma or equivalent; associate or bachelor’s degree preferred.
Previous experience in a healthcare setting, preferably in hospice or home health care.
Excellent organizational and time-management skills.
Strong communication and interpersonal skills.
Proficiency in scheduling software and electronic health records (EHR) systems.
Ability to work independently and as part of a team.
