Job Title : Hospice Scheduler LVN / LPN
Schedule : M-F 8-5 Remote

Scheduling Coordination :
Develop and maintain daily and weekly schedules for hospice staff, including nurses, social workers, and volunteers.Coordinate shifts and on-call assignments to ensure coverage based on patient needs.Communication :
Serve as the primary point of contact for hospice staff regarding scheduling inquiries and updates.Communicate effectively with patients, families, and healthcare providers about scheduled visits and any changes.Data Management :
Maintain accurate and up-to-date records in the scheduling database.Ensure compliance with regulatory guidelines related to scheduling and patient care.Problem-Solving :
Address scheduling conflicts proactively and implement solutions to ensure uninterrupted patient care.Respond to urgent scheduling requests and adjust schedules as necessary.Reporting :
Prepare and present scheduling reports to management, highlighting trends and areas for improvement.Assist in tracking staff attendance and performance related to scheduling.Requirements :
LVN / LPN License from an accredited institution2 Years hospice experience / hospice schedulingHigh school diploma or equivalent; associate or bachelor’s degree preferred.Previous experience in a healthcare setting, preferably in hospice or home health care.Excellent organizational and time-management skills.Strong communication and interpersonal skills.Proficiency in scheduling software and electronic health records (EHR) systems.Ability to work independently and as part of a team.Compassionate and empathetic demeanor towards patients and families.
