Luna Del Valle Hospice is seeking a compassionate and skilled On-Call Registered Nurse (RN) to join our dedicated team. Job Summary: Provides prescribed nursing care services to ill, injured, convalescent and disabled persons in a home setting functioning independently without on-site supervision.

Collaborates with Administrator On-Call (AOC) and/or Call Center staff. Perform Admissions, GIP, CC and routine visits, as assigned. Respond to Triage Nurse to receive patient status information and coordinate follow up including visit activity, coordinating medication refills and physician orders and phone support. Coordinate schedule and care activities with AOC as necessary. May participate in IDG Meetings, facilitate patient conferences, and other patient and families meetings in coordination with IDG and/or Team Director. The employee is free to spend their time as they choose, until they become engaged in work.
Essential Functions:
- Demonstrates the ability to transfer scientific knowledge from social, biological and physical sciences in applying the nursing process.
- Demonstrates knowledge and competency in performing baseline-nursing skills. Maintains clinical competencies according to Hospice standards.
- Administers all medications and treatments according to Hospice policy and procedure, accepted standards of care and Hospice guidelines.
- Communicates effectively with patients, families and physicians.
- Performs accurate assessments, develops and implements a written Plan of Care with the patient, following physician’s orders and according to Hospice guidelines. Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.
- Provide education to patient and caregiver(s) as needed, based on knowledge needs and learning assessment, patient and family condition, in keeping with Plan of Care. Coordinate care with IDG based on the determined Plan of Care. Participate in orientation and ongoing education programs, on committees and special projects and other related duties as assigned.
- Documents patient status in an accurate, timely and concise manner as required by Hospice policy. Addresses patient needs and problems, nursing intervention and patient response, safety interventions and education and teaching.
- Demonstrates organization and leadership abilities by effective use of time and personnel through supervision and teaching. Effectively establishes priorities and delegates responsibilities appropriately when supervising the LVN and the HCA.
- Maintains a committed and cooperative attitude with staffing, promoting teamwork and harmony by readily assisting other staff and patients. Participates in case conferences/team meetings.
- Makes visits as scheduled.
Additional Responsibilities:
- Performs other duties as assigned or requested.
- Possesses adequate knowledge of Medicare/Medicaid and insurance standards to ensure reimbursement for all services rendered.
- Conforms to all applicable Hospice policies and procedures.
- Participates actively in continuing education and inservices.
- Maintains confidentiality of patient information and business trade practices.
- Assumes responsibility for reporting incidents and complaints according to Hospice policy.
Qualifications:
✔ Active RN license in New Mexico
✔ Minimum one year of hospice or palliative care experience.
✔ Strong clinical assessment and critical thinking skills
✔ Excellent communication and interpersonal abilities
✔ Compassionate, empathetic, and dedicated to providing patient-centered care
✔ Ability to work independently and respond promptly to urgent patient needs
