The Kids-A-Part Program at Lund seeks a Community Case Manager passionate about and believes in the power of stability, connection, and advocacy to support children and families impacted by incarceration. The Community Case Manager provides essential support, outreach, and referral services to children and their caregivers, helping them navigate the challenges of parental incarceration. They work closely with local service providers to ensure families receive the resources they need while advocating for parent-child connections when appropriate. The work involves coordinating and supervising mother-child visits, educating service providers statewide, and breaking down systemic barriers that hinder family stability.
As part of a collaborative team and community focus and facing, the Community Case Manager leads the program’s outreach efforts, including training and communication to engage community partners to ensure a seamless transition of support services for children and families.

What You Will Bring:
- Bachelor’s degree in social work, psychology child development, human services or a related field or a combination of relevant work experience required.
- Experience with pregnant and parenting individuals preferred.
- Experience working with teens, children, and families preferred.
- Awareness of the impact of the criminal justice system on children and families.
- Knowledge of substance abuse issues and relapse prevention.
- Familiarity with regulations, laws and procedures of the Vermont Agency of Human Services.
- Ability to maintain a valid VT Driver’s License and access to reliable transportation is required; occasional statewide travel may be required.
