Job Duties:
Responsible for making sure that residents’ physical, emotional, and social needs are meet on a daily basis.
Participate in resident admission process; discharge planning, development, and implementation of social care plans and resident assessments.
Document social service notes in the residents medical charts that are accurate, informative, descriptive, and timely of the services provided and the resident’s response to the services.
Advocate for residents’ rights including providing residents and family members information about resident rights.
Provide consultation to staff members, community agencies, and others as necessary in efforts to solve the needs and problems of the residents through the development of a social services care plan.
Work cooperatively and in collaboration with all departments.
Interview residents and families to obtain social history and evaluate information to assist with determining social care needs.
Provide information to residents and family members regarding Medicare/Medicaid and other financial assistance programs.
Prefer long term care social work experience.
