What are the responsibilities and job description for the Care Plan Coordinator: Registered Nurse position at Lutheran Care Center?
Standards:
- Current Illinois Registered Nurse
- Complete nursing care plans and Minimum Data Sets as required per regulations.
- Infection Control Specialist (or willing to obtain)
- Basic computer skills and knowledge of state regulations
- Ensure compliance with HIPAA regulations and maintain patient confidentiality.
Functions:
- Interview residents on admission and quarterly.
- Complete assessment forms after interviewing
- Knowledge of M.D.S. and complete required sections
- Create care plan for residents and conduct care plan sessions monthly and as needed with residents and family members.
- Complete quarterly wheelchair assessments
- Participate in training sessions, attend UR/Pharmacy meetings and weekly department meetings.
- Contribute to the Nursing Department by working on the floor, any shift, when nursing is understaffed.
- Provide guidance to Nursing Assistants when necessary.
- Strong administrative skills with the ability to multitask and prioritize tasks effectively.
- Excellent patient service skills with the ability to communicate effectively and empathetically.
Job Types: Full-time, Part-time
Pay: $29.00 - $32.00 per hour
