We are seeking a compassionate, organized, and professional Foster Care Case Manager to provide case management services to children and families involved in the child welfare system in Franklin County. Our Foster Care Case Managers work closely with children, biological family and foster parents to address safety concerns, facilitate stronger relationships and help families develop a healthier lifestyle. The ultimate goal is to help each child reunite with their biological family or identify a forever family when reunification isn’t possible.
Our Foster Care Case Managers make a positive impact on the lives of the children and families they work with by providing ongoing support, effective advocacy, education and service coordination. Responsibilities
- Coordinate and provide care to clients that is safe, timely effective, efficient, equitable, and client-centered. Build positive working relationships with clients to meet their goals.
- Assess client’s strengths and obstacles to achieving them. Create a service plan in conjunction with client to develop goals and objectives
- Counsel, advocate, and teach clients on the skills identified in their service plan and provide resources.
- Develop, monitor and evaluate assessments, service plan, and progress as well as any other necessary reports to meet documentation and founder guidelines required.
- Document client progress and provide regular updates in meetings and at court. Maintain client files and ensure they are up to date.
- Establish and maintain professional relationships with community stakeholders and professional team members.
- Respond to clients crisis situations within program parameters in a timely manner.
- Staff in this position will occasionally be “on-call” which includes weekends and holidays.
Qualifications
- Bachelor's Degree in Social Work, Psychology, Criminal Justice, Guidance Counseling, Drug & Alcohol Counseling or other Human Behavior Science.
- Two years’ experience working in child welfare and/or working with children or families is preferred.
- Must be a licensed driver with access to a reliable automobile that is insured in accordance with state and/or organization requirements.
