Your Role:
- Meet clients either face to face, in the community (home/office), or virtually, on a regular basis to achieve direct client service goals.
- Create a service plan in conjunction with client to develop goals and objectives that are consistent with what the client wishes to accomplish.
- Develop, monitor, and evaluate assessments, service plan, and progress as well as any other necessary reports to meet documentation guidelines.
- Continued professional growth and development such as cultural competence, social diversity, therapeutic, and intervention techniques.
What We’re Looking For:
- Case Manager - Bachelor’s Degree in Psychology, Criminal Justice, Guidance Counseling, Drug & Alcohol Counseling, or other Human Behavior Science.
- Social Worker: Bachelor's Degree in Social Work.
- Two years experience with Child Welfare, Adoption, Foster Care, community resources and/or care coordinator.
- Passion for making a difference and supporting families through life’s challenges.
