Lutheran Home of Southbury, a skilled 120 bed nursing facility with the reputation of providing exceptional care and individualized attention to the needs of each of its residents, is hiring a part-time, Licensed Social Worker to assist residents and families in making a smooth transition into the facility.
MUST HAVE:
BSW Degree
Knowledge/experience with:
MDS
PASSR process
Care planning/ documentation
Discharge planning
Assessments
Responsibility:
- Identify the social, emotional, and economic need of patients through a continuous evaluation process.
- Consult with other members of the health care team educating staff on the psychosocial needs of the residents and their families.
- Document and communicate observations and evaluations through reports, records and conferences.
- Develop and serve in a Care Plan for each resident that identifies the needs of the resident. Provide adequate documentation for all services performed.
- Ensure that all charted progress notes are informative and descriptive of the services provided and of the resident's response to the services.
- Intervene in resident conflict situations as needed.
- Requirements:
- One year prior experience in a long term care setting preferred.
- Strong communication and interpersonal skills.
- A Bachelors degree in Social Work.
