Responsibilities may include, but are not limited to:
- Plan, prepare, and carry out individually designed programs of physical therapy care and services to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction.
- Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Identify and document goals, anticipated progress, and plans for re-evaluation in a timely manner.
- Review referrals and medical records to help determine diagnosis and physical therapy treatment required.
- Test and measure patient’s strength, motor development and function, sensory perception, functional capacity, or respiratory or circulatory efficiency and record data.
- Direct and supervise physical therapy assistants in the delivery of physical therapy treatment plans.
- Participate in care plan meetings with other members of the healthcare team as appropriate. Assist with discharge planning and recommendations to facilitate safe transition plans for beneficiaries when appropriate.
- Discharge patients from physical therapy when goals or projected outcomes are attained, or when skilled services are no longer indicated, and provide for appropriate follow-up care or referrals.
- Perform all job responsibilities in accordance with prescribed safety and infection control procedures.
- Follow the guidelines set forth by the Centers for Medicare and Medicaid, the fiscal intermediary, other insurance providers, and the American Medical Association regarding the appropriate delivery of skilled services, documentation required to support these services, and correct billing for medically necessary treatment.
- Adhere to the Physical Therapy Statutes established by the Missouri Board of Healing Arts.

License/Certification required:
- Licensed to work as a Physical Therapist in Missouri
- Experience in a skilled nursing facility preferred
