Lutheran Life Villages, a premier senior living community in Fort Wayne, IN, is seeking a compassionate MDS Coordinator to join our committed team at our Anthony campus. The MDS Coordinator is responsible for overseeing the process of MDS reporting for the facility. Key components of the position are working with Nursing Management and the IDC team to thoroughly capture and report patient information for Medicare and Medicaid billing.
MDS RN pay starting from $36 an hour. MDS LPN pay starting from $30.50 an hour.

Essential Job Functions
- Works in collaboration with the Interdisciplinary Team to assess the needs of the resident.
- Assists with coordination and management of the daily Care Management meeting, to include review of resident care and the setting of the Assessment Reference Date.
- Complies with federal and state regulations regarding completion and coordination of the RAI process.
- Monitors MDS and care plan documentation for all residents.
- Ensures documentation is present in the medical record to support MDS coding.
- Maintains current MDS status of assigned residents according to state and federal guidelines.
- Supports the tracking system of MDS schedules (timeframes and due dates)
- Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs.
- Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members.
- Continually updating knowledge base related to data entry and computer technology.
- Completes electronic submission of required documentation to the State database and other entities per company policy.
- Manages the restorative nursing program
- Assists in the orientation process of new employees
- May be required to cover floor as charge or staff nurse and takes rotating on call status.
- Performs other duties as assigned
Job Qualifications
- Education/Training:
- a) Associate or Bachelor’s Degree in Nursing.
- b) Current licensure to practice in the State of Indiana.
Experience:

- Two (2) years experience in geriatric or long-term care nursing.
- One (1) year experience in supervisory role.
- Two (2) years experience in MDS preferred
- Required Knowledge, Skills, and Abilities:
- Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid Reimbursement.
- Thorough understanding of the Quality Indicator process.
- Knowledge of the OBRA regulations and Minimum Data Set.
- Knowledge of the care planning process.
- Good computer skills
- Strong verbal, organizational, interpersonal and assessment skills
- Ability to organize and prioritize work to meet deadlines
