Description
Position Title: Hospice RN Case Manager
FLSA Status: Exempt
Job Summary:
The Hospice RN Case Manager coordinates and manages the care of terminally ill patients, ensuring they receive comprehensive and compassionate support throughout their end-of-life journey.
Duties and Responsibilities:
- Patient Assessment and Care Planning (25%)
- Conduct initial and ongoing assessments to evaluate patients’ physical, emotional, and spiritual needs.
- Develop personalized care plans in collaboration with the patient, family, and interdisciplinary team.
- Review and update care plans based on changes in patient condition and goals of care.
- Care Coordination (20%)
- Serve as the primary point of contact for patients, families, and caregivers.
- Coordinate services and resources to ensure continuity of care.
- Facilitate communication among healthcare providers and community agencies involved in patient care.
- Clinical Documentation, Reporting and Team Collaboration (15%)
- Maintain accurate and up-to-date clinical records, including patient assessments, care plans, and progress notes.
- Ensure compliance with regulatory standards and hospice policies.
- Participate in interdisciplinary team meetings to discuss patient care plans and progress.
- Collaborate with other team members, including nurses, physicians, social workers, and chaplains, to provide holistic care.
- Patient and Family Education (15%)
- Educate patients and families about disease progression, symptom management, and end-of-life care options.
- Provide emotional support and guidance to help patients and families cope with grief and loss.
- Pain and Symptom Management (10%)
- Monitor and manage symptoms to promote comfort and quality of life.
- Coordinate with physicians and other healthcare providers to adjust medications and treatments as needed.
- Compliance and Training (10%)
- Follow infection control guidelines and maintain privacy rules and regulations as per HIPAA.
- Attend all meetings and complete in-services and mandatory training.
- Provide exceptional customer service and high-quality care by showing compassion and empathy towards patients and their families.
- Performs other related duties as assigned (5%):
Qualifications
Qualifications:
- Education: Completion of an accredited Registered Nurse (RN) program.
- Licensure: Current RN license in the state of practice. Certification in Hospice and Palliative Care Nursing (CHPN) preferred. Must possess and maintain current CPR certification.
- Experience: Previous experience in hospice or palliative care preferred.
- Skills: Strong clinical and patient care skills. Excellent interpersonal and communication abilities. Proficiency in electronic medical records (EMR) systems and office software. Ability to work independently and as part of an interdisciplinary team. Compassionate, empathetic, and dedicated to providing high-quality patient care.
Physical Requirements:
- Ability to lift up to 75 pounds and turn, move, and transport patients using proper body mechanics and lifting devices as needed.
- Capacity to stand, walk, and sit for extended periods.
- Ability to perform repetitive tasks and physical activities, such as bending, kneeling, and reaching.
- Work is performed in a healthcare setting with potential exposure to infectious diseases, bodily fluids, and chemicals.
- Must be able to work various shifts, including weekends, holidays as needed.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Must be able to travel throughout the service area.
