Job Summary
The social worker provides professional services to patients and families, which facilitates the use of individual, public and private resources so that optimal functioning can occur for both the patient and the family. These core services include assessment of psychosocial functioning and capacities, counseling for care planning and decision-making, social work treatment and community resource planning. Social workers collaborate with many disciplines, programs, departments and organizations in the accomplishment of these duties.
Essential Functions
- Maintains list of community resources available to patients.
- Includes families in the planning process.
- Maintains a clear, neat and safe environment for patients and staff.
- Conducts brief and comprehensive psychosocial assessments
- Coordinate with external agencies and stakeholders to enhance service delivery and address client needs effectively.
- Address client concerns and goals while maintaining consistent communication with the client and their team.
- Performs other duties as assigned.
- Complies with all policies and standards.
Qualifications
- Master's Degree Social Work required
- Bachelor's Degree Social Work required
- 1-2 years In healthcare setting preferred
Licenses and Certifications
- LLMSW required
