What are the responsibilities and job description for the MDS Coordinator RN - Full Time position at Lutheran Senior Services?
Summary
The MDS Coordinator RN directs, coordinates, and facilitates the Resident Assessment Instrument (RAI) process, including completion of an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care for residents. This position is an active member of the Nursing Leadership Team, with a focus on staff documentation and resident care.
Responsibilities
1.Performs resident clinical and mental assessment; Collects data consistent with Centers for Medicare and Medicaid (CMS) guidelines and company standards regarding each resident’s problems, strengths, goals, and approaches for nursing care; Uses the RAI process to formulate a pertinent Plan of Care
2. Maintains current MDS status of each resident according to state and federal guidelines as required under the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS, and/or Medicaid required assessment, and implements a system to monitor for changes in resident condition
3. Monitors residents for significant change and opens Significant Change MDS or IPA as applicable
4. Coordinates with Managed Care Companies to facilitate reimbursement from third party payer sources as required
5. Identifies and addresses needs required for assistance with activities of daily living and nursing diagnoses goals and objectives
6. Submits MDS within designated time frames established by regulatory bodies
7. Monitors and reports monthly on Quality Measures to the interdisciplinary team
8. Monitors the MDS scrubber and corrects flags when needed to maintain accurate MDS coding
9. Participates in the development, maintenance, and implementation of the community’s quality assurance program for the Nursing Services Department
10. Participates in survey inspections made by authorized government agencies
11. Makes recommendations to Nursing Leadership, as appropriate, concerning the operation of the MDS/Care Plan process
12. Maintains current, thorough knowledge of MDS 3.0 and subsequent regulatory updates, RAI, and care assessment processes
13. Develops and maintains a good working rapport with the interdisciplinary team
14. Attends and participates in continuing education programs to stay abreast of changes in the profession and maintain nursing license
15. Participates in Nurse Leader duties, including rounding, on-call, staff education, grievance, follow up, and monitoring
Qualifications, Knowledge, Skills & Abilities
To perform this position successfully, an individual must be able to perform each essential duty satisfactorily.
Graduate from an accredited school of nursing
Current RN license in state of practice
Fourth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience.
CPR certified
Ability to make independent decisions when circumstances warrant such action
Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care
Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc. necessary to provide quality care
Ability to exhibit patience, tact, flexibility, a cheerful disposition, enthusiasm, and a willingness to handle challenging residents
