Clinical and Home Care Services Assistant Director
Lutheran SeniorLife provides a comprehensive array of community based health and social service programs throughout western Pennsylvania.
Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.
LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.
Under the direction of the Regional Director of Clinic and Homecare, the Clinical and Home Care Services Assistant Director is responsible for management and coordination of clinical services provided in the center and in the participant home. Assures compliance with all state and federal regulations. Trains and supervises the LIFE clinical and caregiver staff to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants. Provides technical, organizational, and interpersonal skills necessary to effectively and efficiently coordinate the scheduling, communication and documentation of all aspects of the care plan. Responsible for assessment of health needs, creation and implementation of nursing care plans and providing nursing care, as well as, periodic re-evaluation of individual nursing needs. Demonstrates the knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants according to assigned responsibilities and LIFE standards.
Qualifications
for Clinical and Home Care Services Assistant Director:
- A Registered Nurse must be a graduate of a school of professional nursing required. BSN preferred.
